P3.11.01 A STEP FORWARD ON THE PATHWAY TO MATERNAL AND PERINATAL SURVIVAL: GUATEMALA'S MOTHERCARE PROJECT 1994-98 ; E. Bocaletti, Institute of Tropical Medicine, VA, United States Objectives: Guatemala has a Maternal Mortality Ratio of 248 per 100, 000 live births, the third highest in the Western Hemisphere. The majority of births take place at home with traditional birth attendants. The MotherCare Project aimed to bring the communities and health facilities together to save the lives of women and newborns. Methods: Recognition of life-threatening complications and their danger signs and where to go for help were emphasized during TBA training, broadcast by radio to communities, formed the basis for counseling during prenatal care, and were a major focus in the discussions with women's groups. Tutorial training followed by monthly supervisory visits, including sensitization to local birthing traditions strengthened their counseling and clinical skills. Outcome Measures & Results: Rural departments in project areas have witnessed increases in use rates of 50 to 77%. Met Need has doubled in project area hospitals meaning a greater proportion of women having complications seek care ; . Equity has improved through increased use of EOC services by women. The confidence and skills of providers to perform high quality maternal and newborn care have significantly improved, and continues more than 2 years after completion of the training course. The trained providers and facility directors report an improved attitude toward providing care and working in a team, as well as providers' enhanced ability to recognize problems early and to intervene skillfully. Conclusions: Nearly a third of the country has been reached by this community-based approach to reach women and newborns with essential obstetric services. The next steps are to sustain current activities and expand them across of the country, for example, bethanechol 50.

CIRCULATION STATE AND BLOOD OXYGEN SATURATION IN VIBRATION DISEASE PATIENTS L.I. Dmitruk, D.A. Rogatkin, P.N. Lyubchenko M.F. Vladimirsky Moscow Regional Clinical and Research Institute, Moscow, Russia OBJECTIVE: Studying circulation state and blood oxygen saturation in finger skin microvasculature of vibration disease VD ; patients during both dosed cooling and restoration of skin temperature. METHODS: Device "Spectrotest" was used for quantitative estimation of tissue circulation blood volume per tissue unit ; and blood oxygen saturation in 30 patients observed. RESULTS: The mean value of initial circulation in VD patients was 11.00.9 perfusion units pfu ; 16.71.5 pfu in the control group of healthy subjects, p 0.01 ; . After cooling, circulation slightly decreased to 11.461.24 pfu. The mean value of blood oxygen saturation was 86.21.71%, which corresponds to this index value in healthy subjects. After cooling, it decreased to 81.84.99% the difference being insignificant ; . In 20 minutes, saturation returned to initial level but decreased circulation persisted. CONCLUSION: In VD patients, blood circulation in finger skin microvasculature remained reliably decreased as compared with healthy subjects. Medical criteria for prophylaxis initiation have not been uniformly developed and are based on not only migraine frequency but disability as well. Due to the nature of a claims database, qualitative effects of the migraine episodes could not be assessed; therefore, the quantity of triptan medication received was used as a proxy to determine if drug prophylaxis was indicated. This may have inappropriately categorized patients with frequent but less-severe migraines and those with infrequent but extremely debilitating migraines. Although we attempted to control for any variation in migraine severity and frequency by the subgroup definition, it is possible that variations in these factors may not have been fully accounted for. Since these are real-world data not derived from a randomized, placebo-controlled trial, it is possible that selection bias may have occurred. Physicians may have based treatment decisions on clinical factors that could not be captured in the claims dataset. By implementing strict inclusion criteria, we attempted to control for this. Patients included in the subgroup analysis were similar with respect to triptan consumption prior to the initiation of prophylaxis therapy. To control for selection bias, we conducted 2 additional analyses using a model that included a propensity score17 and a model based on matched cases. However, the results remained the same, supporting the methodology used and the fact that appropriate inclusion and exclusion criteria were utilized to identify patients that were comparable. This study examined costs and utilization in a managed care health plan that was a mix of HMO and PPO models, and the results may not apply to other health plans. Finally, this model explains a small proportion 12% ; of variation in migraine-related medical cost between migraine patients. There are other factors that influence overall cost in addition to those we were able to determine from a claims dataset. ss Conclusion Migraine is a costly disorder for health plans. Identification of utilization characteristics is useful in developing disease management programs aimed at increasing quality patient care and decreasing overall costs. Since there is a strong correlation between use in the first 6 months following triptan initiation and the entire first year of follow-up, it is possible to identify high triptan utilizers early in treatment. Continuing triptan patients are much more costly than new starts. It is therefore potentially valuable to a health plan to identify patients who would benefit from an intervention program early following triptan therapy initiation. Patients receiving greater than 18 TEs in a 6-month period may benefit from the use of migraine drug prophylaxis. Migraine drug prophylaxis is cost-saving $559.71 per patient in 1998-2001 dollars ; , and an intervention program that increases the use of migraine drug prophylaxis in potential candidates could be cost beneficial and doxazosin.

The cause of recurrent aphthous ulcers is unknown. They seem to respond well, however, to topical corticosteroids. Lesions that do not respond to corticosteroids should be biopsied to determine their etiology. Cytomegalovirus has been identified in some ulcerative lesions; these may respond to antiviral therapy. The cause of HIV-associated salivary gland disease also is unknown. Clinical symptoms may be alleviated by administration of AZT and anti-inflammatory agents. Salivary flow may be increased by sugarless gums, mints, or candies, or with sialogogues, such as pilocarpine or bethanechol. Artificial saliva products also alleviate dry mouth. Frequent sips of water are recommended. HIV-positive patients with signs of infection in the mouth should receive frequent dental check-ups, at least once every three months. Those without signs or symptoms of oral infection should receive dental check-ups every six months. If Charles is or becomes HIVpositive, it is important for him to maintain a nutritious diet to maintain energy levels, optimize his immune system to fight infection and promote healing of damaged tissues. Vinblastine, under time and dose conditions that inhibited the release of stimulated amylase, also reduced the number of microtubules. The only other consistent structural effects of vinblastine were the presence of vinblastine-induced crystals and an increased incidence of autophagy. The remainder of cell structure was not affected nor were overall tissue A T P and electrolyte contents or the stimulant-induced increase in 45Ca + efflux. It is concluded that the antisecretory effects of vinblastine and colchicine are consistent with a microtubular action, but that acinar cell microtubules are more resistant to the drugs than many other cell types.
