And with five years' experience of managing the pharmacy benefits of an exclusively Medicaid population, AmeriHealth Mercy executives say it is well positioned to move its PBM services to other markets, possibly even those outside the Medicaid arena, such as in Medicare Advantage. Faced with 50 hemophilia cases that cost about $10 million a year, the company used case management along with an aggressive pharmaceutical manufacturer contracting strategy to reduce spending on these individuals to $8 million a year. Carvedilol is a nonselective beta-adrenergic antagonist with alpha-blocking and antioxidant properties, and metoprolol succinate is a beta-1-selective adrenergic antagonist. Both drugs have been shown in post hoc analyses to be beneficial to women with heart failure despite the relatively small number of female participants in each study. Carvedilol has survival benefits in women with moderate heart failure and systolic dysfunction, 53 and appears to reduce the hospitalization rate for women with severe heart failure and impaired systolic function.54 Metoprolol succinate has not been shown to affect survival for women with heart failure and impaired systolic function, but can reduce hospitalizations by 42% P .021 ; .55 It is unclear why metoprolol succinate did not improve survival for women with heart failure and impaired systolic function, since a survival benefit for women was noted with bisoprolol, which is also a beta-1-selective adrenergic antagonist. The reason may be that metoprolol is a less potent beta-1-selective adrenergic antagonist than bisoprolol. Another reason may be that the MERIT-HF study of metoprolol succinate ; 55 included women with a better functional status and a higher ejection fraction than did the CIBIS II study of ibsoprolol ; .56 Since there were few women in the betablocker trials, and many of the trials were terminated early, the beneficial effects of betablockers in women are likely underestimated. ACE inhibitors Current guidelines recommend ACE inhibitors for all patients with symptomatic heart failure and impaired systolic function.38 ACE inhibitors have lowered morbidity and mortality rates in multicenter, prospective, randomized trials.57, 58 Although women participated in most of these trials, the percentage in each study was small, and the benefit of this class of drugs remains unclear even when data are pooled from multiple studies. Garg and Yusuf59 performed a meta-analysis of 30 ACE inhibitor studies involving a total of 1, 587 women with heart failure and found a trend towards a lower mortality rate in women taking ACE inhibitors 13.4% vs.
Patients whose blood pressures are adequately controlled with 50 mg of hydrochlorthiazide daily, but who experience significant potassium loss with this regimen, may achieve similar blood pressure control without electrolyte disturbance if they are switched to bisoprolol fumarate and hydrochlorothiazide and cefaclor and bisoprolol.

Bisoprolol rash
Patients with chronic heart failure CHF ; require careful assessment of their suitability for beta-blocker therapy. Initiation, first prescription and first follow-up should be the responsibility of the specialist. It is then appropriate PTO for GPs to continue to prescribe bisoprolol under the guidance of an ESCA. Licensed Indication: `Treatment of stable chronic moderate to severe heart failure with reduced systolic ventricular function ejection fraction 35%, based on echocardiography ; in addition to ACE inhibitors and diuretics and optionally cardiac glycosides'. Additionally, the Summary of Product Characteristics SPC ; recommends that `the treating physician should be experienced in the management of chronic heart failure'. Clinical Efficacy: Bisoprol9l for the treatment of chronic heart failure has been evaluated in two key randomised double-blind, placebo-controlled trials; CIBIS n 641 ; & CIBIS II n 2647 ; . Patients enrolled in these trials were aged around 60 years old, predominantly male, with NYHA class III heart failure treated with diuretics and ACE inhibitors and digoxin in some cases ; . The planned duration of these trials was 2 years. The CIBIS II trial was however stopped early mean follow up of 1.3 years ; when a significant reduction in mortality with bisoprolol treatment became evident. In the CIBIS II trial, bisoprolol treatment for 1.3 years significantly reduced all-cause mortality RRR 32%; p 0.0001 ; , cardiovascular deaths RRR 25%; p 0.005 ; , & hospital admissions for all causes RRR 15%; p 0.001 ; compared to placebo. These benefits were not apparent in the smaller CIBIS trial. Safety: Adverse events reported in the clinical trials with bisoprolol included dizziness, bradycardia and hypotension. Some patients may have symptoms of worsening heart failure on commencing therapy. Additional Information: At current prices one years treatment costs 125 for bisoprolol 10mg once daily 327 for carvedilol 25mg twice daily Patients should be optimally treated with an ACE inhibitor and diuretic before treatment with bisoprolol is commenced. Close monitoring is required on initiating therapy. Treatment should be introduced at a low dose and increased slowly refer to SPC.
