In this section, we characterize the price that maximizes the firm's profits, P .21 The following proposition summarises the main result: Proposition 2 There are three cases: 1 i ; High coverage. If L R , then P PR 2 and - R consumers purchase the drug. That is only some consumers with reimbursement rights purchase the drug. ii ; Intermediate coverage. If R L , then P PL L and - L consumers purchase the drug: All consumers with reimbursement rights purchase the drug. iii ; Low coverage. If L then P PF 1 and and - F 2 consumers purchase the drug, even consumers with no reimbursement rights purchase the drug.22 insert figure 2 around here profit graph ; Figure 2 provides a graphical representation of the result. The thicker function illustrates the maximum value of the firm's profit as a function of the reimbursement level set by the agency, L ; . The two other functions represent R and F , as defined. In order to understand proposition 2, it is worth acknowledging that the subsidy creates a wedge between the willingness to pay of consumers with reimbursement rights and the willingness to pay of consumers with no such rights. This implies that in order to serve consumers with no rights, the firm must reduce its price substantially so as to make the indifferent consumer F fall below L ; . It will only pay to do so the number of consumers with reimbursement rights is sufficiently low. The shape of the thicker curve in Figure 2 can now be explained. If coverage is large L R , the firm can reach the highest level of profit R as all the consumers who the firm serves have reimbursement rights. However, if coverage is smaller R L ; , it pays for the firm to adjust her prices setting them high ; so as to serve only those consumers who have reimbursement rights. Yet, there is a level of coverage L ; below which it is not profitable for the firm to take notice of the small group of consumers with reimbursement. Abstract: The possibility that non-sedating antihistamines could elicit sedation in mice due to drug-induced inhibition of brain PgP was evaluated by measuring the ability of desloratadine alone or in combination with verapamil to cause ataxia in mice. Also, the concentrations of desloratadine in plasma and in brain homogenates were measured by liquid chromatography-mass spectrometry. Relative to methylcellulose control ; treatment, verapamil plus desloratadine decreased rotarod performance of mice. Plasma concentrations of desloratadine appeared comparable in the mice treated with either desloratadine or verapamil plus desloratadine, however the rate of decline of desloratadine from brain tissue was slower in mice treated with verapamil plus desloratadine compared to mice treated with desloratadine only. These data suggest that inhibition of brain PgP can convert desloratadine to a sedating antihistamine in mice. 1. Simons FER: Antihistamines. In Middleton E Jr, Reed CE, Ellis EF, et al, editors. Loratadine and diphenhydramine hydrochloride

It's chemical name is 8-chloro-6, 11-dihydro-11- 4-piperidilydene ; -5h-benzo cyclohepta pyridine and represented by the structural formula dcl is a metabolic derivative of loratadine, an h-1 histamine receptor antagonist and monistat. These results are broadly similar to the recently reported verapamil-induced 1 mg Kg iv ; increased CNS uptake of dextromethrophan in rats that was also attributed to PgP inhibition [15]. Although the administration of verapamil and also desloratadine by gavage and at higher doses than those used in earlier studies precludes direct pharmacokinetic comparisons, the general outcomes are comparable. The relatively constant plasma concentrations of desloratadine in the initial 60 minutes post gavage are consistent with the 3-6 hr half-life for desloratadine reported in both mdr1 ; mice and wild type mice [4, 16]. In conclusion, high oral doses of desloratadine + verapamil caused ataxia or sedation in mice, though not as profound as a single oral dose of diphenhydramine. Neither desloratadine alone, probenecid alone, nor verapamil alone affected the rotarod performance of mice. While brain levels of desloratadine remained relatively constant for one hour post dosing of desloratadine + verapamil, they appeared to disappear somewhat faster in mice treated with only desloratadine. These findings are consistent with those of others that have shown that verapamil is a weak inhibitor of brain PgP, and can promote the accumulation of dextromethorphan in rat brains [15]. It would be premature to conclude that verapamil and other inhibitors of brain PgP will convert nonsedating antihistamines to sedating antihistamines in humans. However, these experiments using large oral doses of a PgP substrate and an inhibitor in mice, if not proof-of-principle, are consistent with the notion that non-sedating antihistamines can become at least transiently sedating if given with a PgP inhibitor. Both itraconazole and cyclosporine exhibit lower IC50s for PgP inhibition than does verapamil [10], and cyclosporine A has recently been shown to increase brain: plasma ratios of 11C-verapamil in humans [17]. Looking to the future, it might be prudent to consider this type of a drugdrug interaction outcome as the development of potent PgP inhibitors as adjuncts to cancer chemotherapy gains traction. With the current emphasis on drug safety, it's more important than ever for pharmaceutical companies to identify and evaluate their risk management strategies early in the drug development process and plan to continue that scrutiny through the life of the compound, according to an industry expert.
