Neural tube defects NTDs ; are severe, often lethal, congenital malformations of the spine and central nervous system, the most common of which are spina bifida, anencephaly, and encephalocele. NTDs are among the most common and preventable birth defects in the United States. Still, every year in North Carolina, about 1 in every 500-700 pregnancies is affected by an NTD.1 For reasons we don't yet understand, the rate of NTDs is higher in the Southeastern states, including North Carolina, than in the rest of the country. Some specific causes single gene or chromosomal disorders, maternal diabetes, valproic acid use, and hyperthermia ; have been identified, but in most cases no specific etiology is identifiable.2 It is well documented that women who take a daily dose of the B-vitamin folic acid before conceiving can reduce the risk of NTDs by up to 70%.3-6 Because the neural tube closes by the fourth week after conception, it is important that folic acid be consumed on a regular basis before the woman becomes pregnant. In 1991, the Medical Research Council released its findings from a randomized, controlled trial of folic acid, and in the following year the Centers for Disease Control and Prevention CDC ; recommended that all women of childbearing age consume 400 micrograms mcg ; of folic acid daily in order to decrease their risk of having an NTDaffected pregnancy.6-7 In 1996, the Food and Drug Administration required that by January 1, 1998, all enriched grain products be fortified with 140 mcg of synthetic folic acid per 100 g of cereal.8 Because the current fortification level is estimated to increase the average woman's dietary intake of folic acid by only 100 mcg day one-fourth of the recommended amount ; , the CDC, the Institute of Medicine, the.

Monotherapy and 48 patients received divalproex added to other psychotropics Calabrese et al. 1992; this study was an extension of a previous publication, Calabrese and Delucchi 1990 ; . Response rates for mixed manic episodes were quite high at 87%, mania improved in 54%, but only 19% of those with depression were responsive. Hayes 1989 ; reported on 12 bipolar subjects who started divalproex as an add-on medication when significantly symptomatic. After 1 year they experienced an average of 27.7 points improvement on the Global Assessment of Functioning GAF ; Hayes 1989 ; . An interesting study was performed in a group of 30 women who had type II bipolar illness and borderline personality disorder Frankenburg and Zanarini 2002 ; . The patients were assigned to divalproex or placebo in a 2: ratio. Patients receiving divalproex experienced significantly greater improvement in irritability, anger, impulsive aggression, and interpersonal sensitivity Frankenburg and Zanarini 2002 ; . This is similar to what was observed in an open, 8-week study of 11 patients with borderline personality disorder without bipolar illness ; . Only eight patients completed the study and, of these, only three responded with a reduction in anger, impulsivity, rejection sensitivity, and anxiety. Fifty percent of completers experienced an improvement in mood Stein et al. 1995 ; . Combining divalproex with lithium for relapse prevention has also been examined in open studies. Random assignment to lithium alone or lithium plus divalproex was studied using DSM-III-R criteria ; in 20 bipolar patients who were followed for 1 year in an open fashion: receiving combination treatments resulted in a significantly lower risk for relapse but a higher incidence of adverse events Solomon et al. 1997 ; . Similarly, when a group of rapid-cycling bipolar patients treated with lithium plus divalproex were followed for 6 months, 60% experienced marked antidepressant efficacy, and 50% experienced bimodal antidepressant and antimanic ; efficacy Calabrese et al. 2001 ; . Nonetheless, when these patients relapsed, the relapse was usually depressive Calabrese et al. 2001 ; . In summary, vaoproic acid appears to be quite effective in reducing the depressive symptoms associated with acute mixed mania, but less than desirable in its antidepressive effect in depressed bipolar patients. It also appears to be suboptimal in its ability to prevent relapse into bipolar depression, although it is not without efficacy. The data for avlproic acid are educational in that it is clear that efficacy in the depressive symptoms of mania does not predict efficacy in bipolar depression. The two states appear to be biologically different and respond to different treatments.

Concurrent disorders is a term for any combination of mental health and substance use problems. There is no one symptom or group of symptoms that is common to all combinations. The combinations of concurrent disorders can be divided into five main groups: substance use + mood and anxiety disorders, such as depression or panic disorder substance use + severe and persistent mental health disorders, such as schizophrenia or bipolar disorder substance use + personality disorders, such as borderline personality disorder, or problems related to anger, impulsivity or aggression substance use + eating disorders, such as anorexia nervosa or bulimia. other substance use + mental health disorders, such as gambling and sexual disorders. To understand and treat a particular combination, we need to look at the specific problems to see: how severe the problems are how the problems affect each other.
All of the cases developed symptoms after 3-4 days after starting divalproex and olanzapine combination. In 3 cases the symptoms subsided completely after withdrawing olanzapine and symptomatic treatment. Readministration of the offending drug helps to establish the etiology. In our cases rechallenge was not possible as the reactions were severe and patients did not give consent for it. Ryan et al. 2002 ; reported a case of severe stomatitis in a 5-year-old child with pediatric epilepsy 18 months after institution of divalproex sodium, and cessation of the medication was associated with resolution of the stomatitis over 2 weeks. Russo 1981 ; had reported 2 cases of late onset of stomatitis which resolved after 3 days of discontinuation of sodium valproate. One patient developed erythematous tongue within 1 month of valproic acid therapy initiation, and the second patient developed vesicular lesions over tongue and gingival after 3 months of divalproex sodium therapy. One of them was rechallenged and stomatitis recurred. Apthous stomatitis, glossitis and oral ulceration have been described with olanzapine Abdollahi and Radfar, 2003 ; . In apthous stomatitis lesions appear as small, painful, discrete or grouped papules, vesicles which may ulcerate. They appear predominantly over labial or buccal mucosa, and heal without scarring in 10-14 days. Glossitis is inflamma and

Different changes following the unilateral section of 3 to dorsal roots at the cervicothoracical level implicate the development of chronic pain syndrome localised in the ipsilateral limb 2 ; . The brachial plexus is the most common site involved in upper extremity neuropathies in human neonates. Self-mutilation develops in some cases and is severe in patients with total palsy. Unilateral extensive dorsal rhizotomy C4 Th1 ; was performed during postnatal period up to 60 days postnatally ; under pentobarbital anaesthesia 50 mg kg; 2% solution ; in male rats. The frequency of occurrence of self-mutilation behavior was studied. When rhizotomy was performed during the early postnatal period P10 to P22 days postnatally ; , self-mutilation did not develop at all. It is not always essential to make a distinction between thyrotoxicosis due to Graves' disease and that due to toxic nodular goiter, since treatment is usually the same. It is important, however, to consider other diagnoses. As in other age groups, the elderly patient may develop transient thyroid hormone excess secondary to thyroiditis, i.e. destruction of the thyroid with release of pre-formed thyroid hormones 21 ; . Sub-acute thyroiditis should be suspected if the patient complains of sore throat or neck tenderness, typically associated with symptoms of a viral illness or an upper respiratory tract infection. The diagnosis is confirmed by the finding of a raised erythrocyte sedimentation rate ESR ; and absent or very low uptake of iodine-123 or iodine-131.
